Many of the people not understanding how to prepare questions in English
There is very simple way to prepare questions
First learn Meanings, Verb Forms and Question words.
Next learn Helping Verbs and Tenses perfectly, any sentence is forming with Tense, without Tense you can not form any question.
For Example :
Simple Present - No Helping Verbs
you eat. ---------------> This is the Answer

Do you eat? ----------------> This is the Question without Question word.
H.V S V1
You are just changing the places of Subject and Helping Verb.
in answer Subject comes first.
in question Helping Verb Comes first.
In above question we are using "Do" as a Helping Verb. because there are no helping verbs in Simple Present. "Do" is coming as a Helping verb only in Question, not coming in Answer.
Now adding Question word to above question.
What do you eat? ----------------> This is the Question with Question word.
Q.W H.V S V1
What do you eat food? ----------------> This is the Question with Question Q.W H.V S V1 O word and Object.
Present Continuous - am/is/are Helping Verbs
he is eating. ---------------> This is the Answer
S H.V V4
is he eating? ----------------> This is the Question
H.V S V4
You are just changing the places of Subject and Helping Verb.
in answer Subject comes first.
in question Helping Verb Comes first.

Above prepared question using with Helping Verb. Now just add the Question word to Above Sentence.

with added a Question word, Automatically formed One Question.
